Certification Award Policy
Participation in a course does not guarantee the award of a certification or credential.
To be eligible for certification, participants must successfully complete all requirements established by the University of Florida Office of Continuing Dental Education, the course director, applicable accreditation standards, and any relevant state or regulatory requirements. Requirements may include, but are not limited to:
- Attendance at all required course sessions
- Successful completion of written examinations, practical examinations, competency assessments, or other evaluations
- Submission of all required forms, documentation, licenses, permits, or supporting materials
- Completion of any required clinical, laboratory, simulation, or hands-on training components
- Compliance with all course policies, safety requirements, and participant expectations
- Professional and ethical conduct throughout the duration of the program
Certification is awarded at the discretion of the Office of Continuing Dental Education and the course director upon successful completion of all program requirements. The Office reserves the right to deny, withhold, delay, or revoke certification for failure to meet program requirements, provide required documentation, maintain professional conduct, or comply with applicable policies and standards. Attendance, participation, or payment alone does not guarantee certification.
All certification decisions made by the Office of Continuing Dental Education are final.
Certificate Reprint Policy
The Office of Continuing Dental Education maintains records in accordance with applicable University of Florida policies, State of Florida requirements, and ADA CERP accreditation standards. Participants are strongly encouraged to retain copies of all certificates issued to them.
Certificate Requests
A fee will be assessed for printed certificate reprints:
- Continuing Education (CE) Certificate Reprint (1/3 page): $10 per certificate
- Certification Reprint (full page): $25 per certificate
Electronic certificates, when available, are provided at no charge.
Continuing Education certificate verifies Continuing Education (CEU) awarded hours.
Certification includes programs that award a certification, credential, or authorization to perform a specific task or function through the University of Florida or pursuant to State of Florida requirements.
Verification Requirements
All certificate reprint requests are subject to verification. The Office of Continuing Dental Education will review available records to confirm attendance, completion of program requirements, and original certificate eligibility.
As an ADA CERP Recognized Provider, we maintain continuing education records for seven (7) years. Records older than seven years may be available but are not guaranteed.
Important Information
Payment of a reprint fee does not guarantee issuance of a replacement certificate. The fee covers the administrative review and verification process.
Replacement certificates will only be issued when sufficient records exist to verify that the participant successfully completed all requirements and was originally awarded the certificate.
The Office of Continuing Dental Education reserves the right to deny reprint requests when records are unavailable, incomplete, or insufficient to verify eligibility.
